Health Center Web Portal

All new students are required to complete online health forms. These include Consent for Treatment, Health History, TB Screening Questionnaire, Immunization Dates, Medical Insurance Information, and the Notice of Privacy Practices.

The Web Portal is the system for:

access the health web portal

  • completing required medical information forms
  • self-scheduling appointments
  • communicating with Student Health Center
  • viewing and printing a copy of your student health center account
  • viewing and printing a copy of your immunization record

PASSHE Student Health Insurance Plan Option

As stated in BU's recent letter to students, an objective of the university is to ensure students have adequate health care protection. The school sponsored Health Plan is provided by Nationwide Insurance. Please enroll by Friday, Sept. 25.

If you choose to enroll in BU's health insurance plan, you can pay by credit card online or download and print the enrollment form and send in to CHP with a check. If you have any questions, please contact customerservice@consolidatedhealthplan.com.
